We tend to keep leftovers for 1-4 days, then decide whether or not to keep longer (if we haven't already ate 'em) or toss 'em.

Hm, guess this is pretty similar to the first choice, except that we might keep 'em longer, depending on artistic and gastronomic value.

I sweep the fridge every week or two, it's not exact. I mentally track when things were opened/left/stashed, and use that info to sort its contents. If I don't know when an item is from, I'll use my nose, eyes, and deeply cynical nature to decide what to do with the rest.
